Gold Coast ML has partnered with local organisations and the National Heart Foundation to establish
up to 40 walking groups across the Coast and also support workplace walking, to tackle high rates of
obesity and chronic disease
Central Adelaide and Hills ML conducted a citizens jury on the surgical management of obesity,
engaging with a range of community members and medical professionals to produce
recommendations to shape health policy
Proving healthy living and well being are important for people young and old, New England ML
partnered with local exercise physiologists to deliver “Getting your balance” a 14 week program which
included: exercise progressions; weekly education and information sessions which included topics
focused on independent self management and living; and healthy lifestyle habits for improved
wellbeing and quality of life; and falls risk and prevention exercises for the elderly
Central QLD ML initiated a partnership with the Mt Morgan Hospital to deliver Diabetes Education
At Murrumbidgee ML our staff delivering primary health care services, have been trained to deliver
quit smoking brief interventions targeted at groups or individuals
Inner North West Melbourne ML has established a collaborative framework with a local health
network and two local community health services to develop and implement innovative models of care
for patients with type 2 diabetes, chronic kidney disease and low back pain.

Darling Downs South West QLD ML’s Health Services Centre offers services including diabetes
education, chronic disease management, transport services and assistance with prescription collection
for Aboriginal and Torres Strait Islander patients
Perth South Coastal ML has established an office in the rural town of Pinjarra, supporting local
general practice and offering new multi-disciplinary services including diabetes education, access to a
dietician, mental health services and generalist counselling
In collaboration with the Flinders University Rural Clinical School, Country South SA ML is supporting
the ‘Partnerships Exploring Attributes of Coordinated Healthcare Implementation’ (PEACHI) study to
improve chronic disease management and primary care access for patients with intellectual disability.

Townsville-Mackay ML is investing in preventative health programs such as headspace and ATAPS,
the In Home Diabetes Management Trial, New Directions Bubba’s Business and Closing the Gap
Illawarra-Shoalhaven ML and local exercise physiologists and other allied health professionals have
collaborated on the Exercise Is Medicine (EIM) workshop. Clients can benefit from lifestyle and
behaviour changes with a program of targeted exercise and activity. This has also provided a platform
for local primary health care professionals to share their knowledge, particularly with the local practice
nurse networks. ISML is amongst the first Medicare Locals in Australia to host such a workshop.
South Western Melbourne ML practice support team coordinates projects and activities in nursing
leadership with a focus on Nurse-run clinics. Our team includes a diabetes educator who works with
practices to advocate a systematic approach to Chronic Disease Management, including Diabetes

Greater Metro South Brisbane ML’s Positive Impact program targeted toward consumers at risk of
chronic disease has enrolled 554 participants in the phone coaching service. The age range of
participants is 18-84 years and the average BMI of participants is 37.5. The total weight participants
lost between January to September 2013 is 1670kg. Across the lifespan of the program GMSBML has
engaged 373 general practitioners to refer to the program from 103 practices.
